In order to qualify for an adult deed poll you must be at least 16 years of age and living in the United Kingdom, or be a British citizen. Child deed polls

Applying to change the name of a child is slightly more complex than applying for an adult, though it is still relatively simple. You will need to have parental responsibility for the child for who you are applying, and everyone else with parental responsibility will also need to consent to the name change.

Before we can release a child deed poll we also need to satisfy ourselves that you have the permission of everyone who has parental responsibility for the child.
