Do I need a deed poll to revert to an old name?
Yes. Whenever you change your legal name, whether it be to a completely new name or reverting to a previously used name, you must provide documentary evidence of the change.
For example, when a woman wishes to revert to the name she had before marriage (her maiden name), she must provide her decree absolute certificate to prove that she is no longer using her married name.
The same rule applies to those that have changed their name by deed poll. If you've executed a Deed of Change of Name (deed poll) in the past and now want to revert to the name you held previously, you'll need to execute another deed poll.
